Combination training is more effective than singular modalities in enhancing physical and psychological status in male soccer players with anterior cruciate ligament reconstructions

1. Abstract
Assess traditional training (CON), eccentric-only training (ECC), plyometric-only training (PLYO), or a combination of these two (COMB,) in ACL rehabilitation. National level male athletes rehabilitating from ACL reconstruction, randomized into each group, training twice weekly for six weeks. Y-Balance test, quadriceps circumference, quality of life and return to sport index were measured. There were significant group x time interactions for all variables (p<0.001). Paired t-tests revealed significant changes in all variables (p=0.0002-0.0006) for the COMB group which also demonstrated the largest effect sizes for all variables (d=4.1-13.0). For PLYO, there were significant changes in all variables (p=0.0002-0.02) with very large effect sizes for all tests (d=6.81-7.29), except for thigh circumference (d=0.96). The ECC group also demonstrated significant changes in all variables (p=0.006) but demonstrated inferior effect sizes to the PLYO and COMB groups with respect to QOL and RSI (d=6.1). The CON group achieved significant changes in only three of the four measured variables (p=0.003-0.006) and had the lowest effect sizes except for thigh circumference (d=3.1). COMB training is a multidimensional stimulus which has physical and psychological benefits in the post-surgical period.

3. Introduction 
Recent advances in rehabilitation following anterior cruciate ligament (ACL) surgery continue to progress with current emphasis being on patients achieving immediate weight-bearing [1,2], range of motion [2], progressive muscular strengthening [3], dynamic stability and neuromuscular control through exercise [4]. Subsequent to surgery, patients engage in extensive rehabilitation programs with a view to enhancing the strength, stability, flexibility and muscle power of the injured limb [5]. Usually, a four to nine-month rehabilitation program is implemented after surgery [6]. However, there is still some controversy about the optimal protocol that practitioners should prescribe. Though females possess a greater predisposition to sustaining non-contact ACL injuries than males [7], injuries remain common in both sexes [8]. Despite males being less vulnerable to this type of injury, the associated risk factors are the same as they are in females with low knee flexion angles, greater knee valgus and high ground reaction forces upon landing some of the key variables [9]. Despite these risks a multicomponent training program can offset the risk of sustaining ACL injuries [10]. 
A standard rehabilitation program typically involves concentric and eccentric exercises [11] with a recent critical review [12] indicating that eccentric training, such as Nordic hamstring curls, may be most effective in restoring hamstring strength. Indeed, in patients (n=33) with autogenous hamstring ACL reconstructions, Kinikili et al. [6] demonstrated greater improvements in measures of vertical jump (p=0.012), single-leg hop (p=0.027), Lysholm knee scale (p=0.002) and quality of life (QOL) (p=0.000) after 12 weeks of progressive eccentric and concentric training. It has also been suggested that neuromuscular training, incorporating motor learning principles, which requires patients to demonstrate newly acquired skills after initial practice, can be combined with strength training to optimize outcomes [12].

Physical rehabilitation programs tend to start immediately after surgery and last from twelve to 24 weeks, supporting the regeneration of musculotendinous tissue at harvest sites [14]. However, it is only three to four months after reconstruction that the focus of such rehabilitation programs shifts to the enhancement of muscular volume and strengthening [15]. Further to this, muscle morphology [16], balance and stability [5] and psychosocial status [3,6] are considered to be the components tested most often during rehabilitation; but, despite this, little is known about morphological changes, QOL and functional performance alterations in the quadriceps femoris muscle after different combined rehabilitation programs. Friedmann-Bette et al. [16] conducted twelve weeks of standard rehabilitation following ACL reconstruction in recreational athletes (n=37). After the training period, the researchers deployed a twice-weekly program of either conventional eccentric or eccentric overload resistance training with additional load, using one-legged supervised leg-press training. The authors found that eccentric overload resistance training lead to significantly greater muscle hypertrophy than conventional eccentric resistance training; however, at the same time, induced a slower muscle phenotype for strong and fast movements. With this in mind, further study using more advanced exercise such as eccentric, concentric and combined modalities is required to develop a relevant training protocol of appropriate content and intensity. Thus, the aim of this study was to assess the effect of six weeks of traditional rehabilitation training, eccentric-only training, plyometric-only training, or a combination of these two modalities, on measures of dynamic balance, quadriceps muscle circumference, QOL and return to sport index (RSI) in national level male athletes. We hypothesized that the combined condition would lead to greater improvements in the outcome measures of interest.
4. Materials and Methods
Subjects
Forty male athletes, who were rehabilitating from ACL reconstructions, participated in the study. The participants had a mean age of 20.4 � 3.0yrs (range 17-28yrs), mean height of 180.9�7.4 cm (range 178-183 cm), and mean weight of 77.8 � 7.0 kg (range 72-83 kg). Subjects with high blood pressure, hemophilia, heart disease, sickle cell trait, an acute infection, edema in the limbs, a blood/bleeding disorder or a skin disorder on a lower extremity were excluded from the study. Athletes who were taking anti-inflammatory drugs or muscle relaxers were also excluded. To be included in the study, participants: had to have been released back to activities of daily living by a healthcare professional (self-reported); had to have had one surgical knee intervention for an ACL injury 12 weeks prior to partaking in the study; had to have had undergone an ACL rehabilitation program and had to have had no self-reported anterior knee pain. All patients were operated on with the same surgical procedure that being a patellar tendon bone graft. There were 26 individuals who underwent surgery on their right limb, and 14 had surgery on their right limb. Prior to partaking in this study, all participants provided written informed consent for their involvement and this was approved by the institutional Scientific Research Ethics Committee.
Design and procedures 
Athletes (n=40) from multiple different sports, who received ACL reconstructive surgery, via the same surgical methods (Kenneth Jones or Bone Patellar Tendon Bone (BPTB)), and the same surgeon, participated in this six-week intervention. All were under care at the National Center of Medicine and Science in Sports, Tunisia (2.5 hrs per day, 5 days per week), using the same periodization scheme and the same physical therapist. The participants were randomized into four groups of ten. They included the control group (CON) which was rehabilitated according to a traditional [17,18] from Monday to Friday (5 Days). The plyometric group (PLYO) took part in the traditional protocol on Monday, Wednesday and Friday, and included plyometric work on Tuesday and Thursday. The eccentric group (ECC) took part in the traditional protocol on Monday, Wednesday and Friday, and eccentric work on Tuesday and Thursday. The combined group (COMB) undertook the traditional protocol on Monday, Wednesday and Friday and either plyometric work or eccentric work on Tuesday or Thursday respectively (volume matched).
Training program
A progressive resistance training program was conducted twice per week (Monday and Thursday or Tuesday and Friday, respectively), for six weeks. It was carried out under the supervision of a certified rehabilitation coach. Before each training session, the participants completed a standardized warm-up program and exercises for the improvement of coordination. The programs followed by each group are displayed in (Tables 1-4).
Before starting the experiment, and prior to the pre-training testing session, all players completed a two-week orientation period (three sessions per week) to become familiar with the general environment and technique of the utilized fitness tests. During this orientation phase, all participants received consistent instructions on proper technique for the exercises from certified strength and conditioning specialists.
Dependent variables
Assessment of lower limb muscle volume:
Muscle volume of the lower limb (thigh) was calculated before and after the training period and was based on leg circumference from the base of patella (kneecap) to greater the trochanter (approx. 15 cm). The length of the leg was measured from the anterior superior iliac spine (ASIS) to the medial malleolus. The length and circumference were measured to the nearest 1 mm using a flexible standard measuring tape. Skinfold thickness was measured using skinfold calipers (Beta Technology Incorporated, Cambridge, MD) at three sites at the midpoint of each limb segment. Specifically, for the thigh, these sites were medial, anterior, and lateral, while for the lower leg, medial, posterior, and lateral sites were used [19]. 

Dynamic Balance
Dynamic balance was assessed using the Y-Balance test (Y-BAL) as described by Shaffer et al. [20]. For each trial, subjects placed their hands on their hips and began in a unilateral stance with the most distal aspect of their big toe behind the line on the center of tape positioned on the ground. Distances were then recorded by pushing a target reach indicator in three different directions, with trials being performed on the dominant leg. Throughout, subjects were required to keep the heel of the non-reaching leg on the testing platform, maintain balance in a single leg stance, and return the reach foot back to the start prior to attempting the next direction. To ensure accuracy, no kicking of the target reach indicator was permitted. Maximal reach distances were recorded to the nearest 0.5 cm marker on the test kit. Balance performance was calculated as the YBT composite score (MADX%), obtained by dividing the sum of the maximal reached distances in the three directions by three times the length of the lower limb. The lower limbwas measured from the most distal end of the anterior superior iliac spine to the most distal end of the medial malleolus and was then multiplied by 100: MADX%= {[(A + PM + PL) / (LL � 3)] � 100}.

Quality of life 
We assessed QOLusing the anterior cruciate ligament-quality of life instrument [21].This was developed to assess the QOL of patients presenting a chronic traumatic lesion.It comprises of 31 elements which are divided into five different domains: symptoms and physical complaints, work-related concerns, leisure activities and participation in competition and / or sports training, lifestyle and social conditions and emotional aspects [21]. The final score ranges from 0 to 100 points, with the highest score representing the best quality of life [22]. The tool is considered valid and reliable [23].
Return to sport index
The RSI questionnaire considered three elements that have previously been correlated with returning to sport: emotions, confidence in one�s performance and evaluation of risk. It comprised of twelve questions anchored to a visual 10-point Likert scale, in the form of boxes to be ticked, ranging from 0 to 10. The total score was obtained by adding the values of the twelve responses, then obtaining their relationship to 100 to obtain a percentage, with high scores corresponding to a positive psychological response [24]. The tool possesses a high degree of internal consistency (�=0.96). 
5. Statistical Analysis
Statistical analysis was carried out using IBM SPSS (version 25). Data normality was determined with the Shapiro Wilk test. A repeated-measures ANOVA test was used to detect statistically significant (P<0.05) between-group differences. Post-hoc analyses were conducted with paired t-tests tests to determine within-group changes from baseline to follow up. Cohen�s d effect sizes (ES) were classified as �trivial� (<0.2) �small� (>0.2-0.6), �moderate� (>0.6-1.2), �large� (>1.2-2), or �very large� (>2) [25].
6. Results
As data were predominantly normally distributed, they were analysed with parametric tests. The results for the intervention are displayed in Tables 5-8. The repeated measures ANOVA revealed significant group x time interactions for Y-BAL (p<0.001), RSI (p<0.001), QOL (p<0.001) and thigh circumference (p<0.001). The paired t-test revealed significant baseline to follow-up changes in Y-BAL (p<0.001), RSI (p<0.001), QOL (p<0.001) and thigh circumference (p<0.001) for the COMB group which also demonstrated the largest effect sizes for all variables (d=4.1 to 13.0). In the PLYO group, there were significant changes in all variables (Y-BAL: p<0.001; RSI: p<0.001; QOL: p<0.001; thigh circumference: p=0.005) with very large effect sizes for all tests (d=6.81 to 7.29), with the exception of thigh circumference (d=0.96). The ECC group also demonstrated significant changes in all four dependent variables (Y-BAL: p=<0.001; RSI: p=<0.001; QOL: p=<0.001; thigh circumference: p=<0.001) but demonstrated inferior effect sizes to the PLYO and COMB groups with respect to QOL and RSI (d=6.1) but achieved the second highest effect size for thigh circumference (d=1.6).The CON group achieved significant changes in all four measured variables (Y-BAL: p=<0.001; RSI: p=<0.001; QOL: p=<0.001; thigh circumference: p=0.037) but had the lowest effect sizes across all tests, with the exception of thigh circumference (d=3.1). 
7. Discussion
The most important findings of the present study were that with a six weektraining program for athletes who have rehabilitated from ACL reconstruction, using a combination of eccentric and plyometric (COMB) exercises, was effective in improving dynamic balance, return to sport index (RSI), QOL and thigh muscle volume. In eliciting these changes, COMB was more effective than PLYO, ECC and traditional rehabilitation, carried out in isolation. In accordance with the concept of training specificity [26], it might be expected that a program that included combined PLYO and ECC activities would improve outcome measures to a greater extent than these training types would alone. In support of this, a number of studies have demonstrated improved performance-related test scores with COMB programs after ACL reconstruction [6,16,27].
In our study, we found an improvement in thigh muscle volume, with the largest significant effects observed in the COMB group. This seems to show the adaptive potential of muscle morphology in athletes after ACL reconstruction. Morphological changes following strength training include an increase in muscle fiber size and changes in fiber-type composition and connective tissue, in addition to structural changes in the muscle itself [6]. Friedmann-Bette et al. [6] demonstrated that strength training with an eccentric overload lead to significantly greater muscle hypertrophy than conventional strength training. The authors exposed recreational athletes (n=68) to twelve weeks of strength training after which they observed that the eccentric group achieved greater increases in fiber cross-sectional areas of all muscle fiber types, displaying a significant correlation with increases in muscle strength. The addition of an eccentric overload in this case corroborates our findings with the ECC group achieving the second highest increase in thigh circumference, an adaptation that was enhanced in the COMB group which added plyometric exercise to the rehabilitation protocol. The combination of these results suggests that different mechanisms play a role in contributing to a comprehensive rehabilitative with increases in muscle mass and strength being seemingly bolstered by the additive effect of plyometric training which taxes the neuromuscular system and associated stretch shortening-cycle capabilities [28].
Substandard movement mechanics, such as excessive lateral displacement of the trunk or asymmetric limb loading, play an influential role in creating valgus forces at the knee joint and exposing that part of the body to a higher risk of injury [29]. Though this is a greater risk in females [30], targeted neuromuscular training to offset this risk is still an important to consider in male populations [31]. Neuromuscular training is a wide-ranging term and can be characterized as facilitating sudden movement-based responses during rapid changes in the positioning of a joint [32]. The term encapsulates plyometric exercise which can be highly beneficial for postural control and dynamic stability when it induces unconscious efferent responses to afferent stimuli [32]. Accordingly, our results revealed that the injured limb displayed greater performance in the Y-Balance test after COMB than after ECC, PLYO or CON training, which were all nonetheless highly effective in this regard. However, it seems that the addition of neuromuscular exercise to the rehabilitation protocol was additive in this case, underlining the efficacy of the COMB protocol. Plyometric training comprises of a dynamic form of resistance training involving a rapid stretch-shortening cycle which can involve both vertical and horizontal displacement of the center of gravity, improving an individual�s anticipatory postural adjustment [35]. Anticipatory postural adjustment challenges result in proactive [36] or feed-forward [37] adjustments that stiffen involved musculature, prior to landing. Furthermore, the sensitivity of afferent feedback pathways can be improved with balance and motor skill training [38], resulting in a faster onset of stability [39]. The dynamic nature of the COMB exercises could have enhanced motor control and leg stiffness which, in turn, could improve postural control and balance [39]. 
While, to our knowledge, there are few, if any, studies on COMB protocols after ACL surgery with which to compare our results, several investigations have focused on PLYO and ECC protocols separately in rehabilitation after ACL reconstruction. The International Knee Documentation Committee Subjective Knee Form Documented Data indicates the effectiveness of plyometric exercises in improving QOL [40] with post-intervention outcomes approaching those of patients who returned to sport one year after surgery [41]. Despite this, it has been put forward that the onset of early osteoarthritis can occur within eight to 15 years of reconstructive ACL surgery, potentially jeopardizing the career and subsequent health of any athlete that sustains an injury [42]. Accordingly, the execution of strength and plyometric training may be an important determinant of QOL as an injured individual ages [42]. For instance, plyometric training has been associated with positive outcomes relating to bone mass [43]. Indeed, as individuals advance in age, the advantage of combined resistance and plyometric training over plyometric training only, is emphasized, as in the current study [43]. This seemingly facilitates a more comprehensive adaptation by way of differing adaptive responses to the respective exercise protocols. On this, it was previously demonstrated that plyometric activity enhanced eccentric force production in the lower body to a greater degree than traditional resistance training which predominantly favoured concentric adaptations [44]. Accordingly, the combination of these activities is important as muscles must act eccentrically to decelerate the body when moving, and to store elastic energy for reutilization in subsequent concentric actions [45]. Further to the above, the present study demonstrated that the COMB group outperformed the ECC and PLYO groupson RSI scores. This could be because the protocol of the COMB group imposed a more comprehensive stress of greater magnitude and improved psychological status by way of increased confidence levels for the athlete. This assertion is supported by Ardern et al. [1] who highlight the discrepancy between the function required to return to sport, and that required to resume full physical capability. It is plausible that the difference between these two states is representative of the gap between confidence to play, and a lack thereof, in the post-operative period with fear of reinjury an influential factor in an individual�s psychological state [1]. It is possible that the comprehensive nature of a COMB training stimulus is beneficial in this regard as it may provide athletes with the confidence they require to return to competitive play. The relatively low rate of return to competitive sport, despite high rates of successful surgical outcomes,supports the use of psychological interventions as being part of a multidimensional approach to ACL rehabilitation, alongside physical activity programmes that closely reflect the demands of the athlete�s sport.
8. Conclusion
Based on our results it is apparent that, despite all of the training methods inducing successful outcomes to various extents, COMB seems to be the most effective protocol to stimulate positive changes in dynamic balance, thigh circumference, QOL and RSI index. On this basis, COMB seems to represent the most multidimensional stimulus as it exerts positive effects on both physical and psychological metrics of rehabilitation status in the post-surgical period. This could result in a synergistic response to rehabilitative exercise whereby an athlete�s confidence is underpinned by concurrent progress in physical and psychological domains, ensuring the greatest chance of achieving personal wellbeing as recovery from injury is addressed. We encourage rehabilitation professionals to adopt multidimensional programs which address, specifically, eccentric strength, muscle power, coordination and balance. It seems a COMB programme is a credible method to achieve these aims.
