Abstract
Background: Reports indicate that top-class compared to less well-trained sailors demonstrate greater hiking abilities by sustaining greater fractions of their maximum quadriceps muscle strength.
 Aim: To investigate the extent to which central hemodynamic and local muscle capacities for oxygen transport and extraction are superior in highly-ranked compared to less well-ranked sailors. 
Methods: Six highly-ranked and six club sailors performed 3 successive 2-min hiking bouts followed by a 4th bout to exhaustion. Vastuls lateralis muscle deoxygenated hemoglobin concentration (an index of local muscle oxygen extraction) was measured by Near-Infrared Spectroscopy, whereas cardiac output was determined using impedance cardiography. 
Results: Highly-ranked compared to club sailors demonstrated significantly prolonged hiking endurance time (298�10 versus 201�7 sec). Mean arterial blood pressure and quadriceps muscle oxygen extraction, were greater in highly-ranked as compared to club sailors throughout the three successive hiking efforts. At the limit of tolerance of the final hiking bout, cardiac output, stroke volume, and systemic vascular conductance were significantly greater in highly-ranked (26.1�1.2 l/min; 180�10 ml/beat; 179�5 ml/min/mmHg, respectively) compared to club sailors (21.7�1.1 l/min; 131�7 ml/beat; 160�7 ml/min/mmHg, respectively). At the end of the final bout, highly ranked compared to club sailors exhibited significantly greater quadriceps muscle deoxygenated hemoglobin concentration (23.2�2.7 �mol/l versus 15.5�1.6 �mol/l, respectively).
Conclusions: Highly-ranked sailors exhibit superior cardiovascular and quadriceps muscle oxygen utilization capacities. Such superior central and peripheral muscle capacities for oxygen transport and utilization may justify why highly-ranked sailors demonstrate advanced hiking ability.

Introduction
In dinghy sailing, hiking is the action of moving the sailor`s body weight as far to windward as possible, in order to withstand the heeling moment on the boat. This can mainly be achieved by hooking the feet under toe-straps attached to the bottom of the hull, supporting the back of the legs on the deck and suspending the rest of the body over the water. Thus hiking is the ability to counterbalance the heeling moment which is created by the wind that blows into the sail. In addition to the most optimal reaction ability, also anticipation on wind gusts that can be seen on the water defines an optimal hiking technique. In fact, it is the accuracy in which sailors counterbalance the heeling moment that defines successful sailors (1-4). 
Dinghy hiking constitutes a major physical challenge as the effort of holding the boat upright in strong winds requires a �quasi-isometric� contraction of the quadriceps (4), abdominal and other trunk muscles for periods typically ranging from 11/2 to 4 min (1-5). Measurements of electromyography activity in quadriceps muscles during hiking have shown that the required isometric effort progressively increases from 30 to 50% of maximal voluntary contraction (MVC) (6, 7). However, on-water hiking studies show the same intensity on flat water with the development of greater intensities (i.e. 50-75% MVC) dependenting on the wind force with peaks possibly up to 100% MVC (2).Simulation studies have shown that at such high fractions of isometric MVC, highly-ranked sailors sustain hiking significantly longer compared to club sailors, thereby suggesting that the best sailors exhibit enhanced cardiovascular and/or local muscle adaptations that allow more efficient transport and utilization of oxygen (7).
Along these lines, studies have shown that the development of high intramuscular pressure, resulting from the prolonged isometric nature of hiking (8), limits quadriceps muscle blood flow and causes reduction in local muscle oxygen availability that is reflected by the reduction in local muscle oxygen saturation and increase in deoxygenated hemoglobin concentration (9, 10). Indeed, a recent study from our laboratory (10) documented that in a mixed group of highly-ranked and club sailors, the progressive reduction in quadriceps muscle oxygen saturation during hiking was due to limited blood flow to these muscles. In fact, the increase in vastus lateralis blood flow during hiking was insufficient to meet the demands for oxygen and as such there was a significant increase in local muscle tissue oxygen extraction reflected by greater deoxygenated haemoglobin concentration (10).
However, in our previous studies (9,10) there was not a sufficient number of highly-ranked and less successful sailors to identify the type of adaptations taking place at systemic and/or local muscle level allowing the top class sailors to hike longer compared to the average competitive sailor. In view of that, it was thought that studying sailors with different sailing ability according to the national ranking list would allow us to differentiate the potential effect of training adaptations on systemic and /or local muscle oxygen availability during hiking. 
Accordingly, the aim of the present study was to investigate the extent to which central hemodynamic responses and quadriceps local muscle oxygen availability during repetitive hiking bouts differ between sailors with different sailing ability according to the National Laser sailor�s ranking list and years of experience. The study by Vangelakoudi et al. (7) reported a greater endurance time for highly-ranked sailors, due to a higher intramuscular coordination capacity. Tan et al., (3) reported further that maximal strength capacity was greater in highly-ranked sailors. Combining these 2 findings, we could assume that highly ranked sailors have a greater endurance time compared to their less well trained counterparts due to the fact that they are capable of sustaining substantially high fractions of their maximum quadriceps muscle strength for prolong periods of time. Accordingly, it was reasoned that highly-ranked compared to club sailors would exhibit greater central hemodynamic responses and local muscle oxygen utilization capacity, allowing them to sustain hiking for longer.

Materials and methods
 Subjects
	Twelve male Laser-Class National Squad sailors (age: 23�2 yr; stature: 178�3 cm; body mass: 77�4 kg) participated in the study which was approved by the authors� University Ethics Committee. All subjects gave written signed informed consent and instructed to avoid heavy exercise 48 hours prior the test. The characteristics of the highly-ranked and the club sailors are given in table 1.
Experimental design
	The test conducted on a Laser ergometer (7, 9, 10) in one visit. Each participant performed three successive 2-min hiking bouts followed by one hiking bout to the limit of tolerance. Between the successive bouts, 5-s rest intervals were used to simulate tacking. Quadriceps muscle oxygenation responses were recorded continuously throughout the whole testing period.
Hiking bouts 
The characteristics of the Laser simulator used in the present study have been described in detail elsewhere (6). During hiking, participants were required to maintain constant the force exerted on the toe-strap and reproduce this force throughout all four bouts. Force was measured by a load-cell amplifier (attached to the toe strap) which monitored the force applied and recorded on a computer program which provided visual information to the subjects to assist them in maintaining the simulator level and the force exerted on the strap constant. Simulation of tacking involved sitting in from the hiking position and 5 seconds later moving back out to full a hiking posture. On a different day, subjects were required to visit the lab to perform a sequence of 3-5 maximal hiking efforts to establish the maximal hiking moment in Nm. This was necessary so as to express the individual sailor�s average moment during hiking bouts as a fraction of maximal hiking moment. The choice for the duration of the three successive hiking bouts was to some extend similar to previous studies performed on the same dinghy simulator, where electromyography activity from the quadriceps muscles indicated that hiking was sustained initially at 30% and progressively at 50% of maximum voluntary contraction (MVC) (6, 7). Therefore to reproduce the results of previous studies (6, 7, 9, 10) a similar  protocol was employed. However in the present study we performed an additional bout to the limit of tolerance to define endurance hiking capacity. 
Body composition
Body composition was assessed by single-frequency bioelectrical impedance (Maltron BF 907 body fat analyzer, UK). All bioelectrical analysis measurements were performed 2-3 h after the last meal and prior to the study protocol to avoid changes in hydration, skin temperature, plasma electrolyte concentration and glycogen stores (11). The FFM was calculated using the below equation which has been specifically validated in healthy subjects (12). 
FFM = -4.104 + (0.518 x height2/ resistance) + (0.231 x weight) + (0.130 x reactance) + (4.229 x sex ) [males=1, females=0]
Cardiac output
	An impedance cardiography device (Physioflow PF05L1, Manatec Biomedical, Macheren, France) was used to determine heart rate (HR), stroke volume (SV), and thus cardiac output (CO) at rest during the hiking bouts and in recovery. The Physioflow device uses changes intransthoracic impedance in response to an administered electrical current during cardiac ejection to calculate SV as has been described elsewhere (13-17). The device emits a high-frequency (75 kHz) and low-amperage (3.8 mA peak-to-peak) alternating electrical current via skin electrodes ( ADDIN EN.CITE  ADDIN EN.CITE.DATA 13-17). Two pairs of electrodes, one transmitting and the other receiving, are applied above one another so as not to overlap at the supra-clavicular fossa at the left base of the neck and at the midpoint of the thoracic region of the spine. An additional pair of electrodes is used to monitor a single electrocardiogram lead (V1/V6 position). Before each constant-intensity exercise test, the system was auto calibrated. The auto calibration procedure was started after a period of at least 5 min, in which participants were sitting immobile on the cycle ergometer.
Quadriceps muscle oxygenation by NIRS
	Skeletal muscle oxygenation profiles of the left vastus lateralis were evaluated with a commercially available near-infrared spectroscopy system (NIRO 200; Hamamatsu Photonics KK, Hamamatsu, Japan). In order to measure quadriceps muscle oxygenation, a set of near-infrared spectroscopy (NIRS) optodes were placed, over the left vastus lateralis muscle 10-12 cm above the knee, secured using double sided adhesive tape. The optode separation distance was 4 cm, corresponding to a penetration depth of ~2 cm. The variables assessed by NIRS were the concentration changes of oxygenated, deoxygenated and total haemoglobin. All peripheral muscle oxygenation parameters were evaluated as the change from baseline. The deoxyhemoglobin concentration ([HHb]) signal (�M/cm) during exercise has been considered as a proxy of fractional O2 extraction in the microcirculation, reflecting the balance between oxygen delivery and utilization (18-21): 
Oxygen delivery =oxygen utilization (VO2) divided by fractional oxygen extraction (i.e., at a given level of VO2, oxygen delivery and fractional O2 extraction are expected to be inversely related) (22)


Statistical analysis
	Data are presented as mean � SEM. The minimum sample size was calculated based on 80% power and two-side 0.05 significance level using the standard deviations from our previous study (10). The required sample size foe each group (i.e.: highly-ranked or club sailors) was calculated to be six participants. Differences in subjects characteristics and hiking time, between highly-ranked and club sailors were tested by Independent sample t-tests. Two-way analysis of variance (ANOVA) with repeated measures was used to identify statistically significant differences across the four hiking bouts between the two groups, for all variables. One-way ANOVA with repeated measures was used to identify statistically significant differences within each group across the four hiking bouts for all of the aforementioned variables. As appropriate, when one or two way ANOVA detected statistical significance, pair-wise differences were identified using LSD post hoc procedure. Comparisons for all variables calculated only during the fourth bout of the hiking test protocol were tested by Independent sample t-tests. The level of significance for all of the above analyses was set at P < 0.05.


Results
Anthropometric and performance� characteristics 
Table 1. displays subjects characteristics for the two groups. While age, height and body fat did not differ among groups, body mass, body mass index as well as fat free mass were higher (p<0.05) in highly ranked-sailors compared to club sailors. In addition, maximal hiking moment was greater in highly ranked-sailors compared to club sailors (p<0.05). Moreover, highly ranked-sailors were more experienced compared to club sailors. According to National ranking list highly ranked-sailors were classified between 1st and 7th top positions, whilst club sailors were classified between 11th and 24th position. Highly-ranked sailors sustained the final hiking maneuver to the limit of tolerance significantly (p=0.0001) longer (298�10 sec) compared to the club sailors (201 �7sec).
Maximal hiking moment
The average hiking moment in absolute values (Nm) that was sustained throughout the four hiking bouts was significantly greater in highly-ranked compared to club sailors (Figure 1A). However, when the average hiking moment was expressed as a fraction of the predetermined maximal hiking moment did not differ between highly-ranked and club sailors throughout the hiking bouts (Figure 1B). 
Quadriceps muscle oxygenation responses
Highly-ranked sailors exhibited a greater decrease in vastus lateralis muscle oxygenated hemoglobin concentration (Figure 2A), greater increase in vastus lateralis muscle deoxygenated hemoglobin concentration (Figure 2B) as well as greater reduction in vastus lateralis muscle oxygen saturation (Figure 2C) throughout the three 2-min hiking bouts. At the limit of tolerance, vastus lateralis muscle oxygenated hemoglobin concentration (Figure 2A) and fractional oxygen saturation of vastus lateralis muscle (Figure 2C) were significantly lower, whilst vastus lateralis muscle deoxygenated hemoglobin concentration was higher in highly-ranked compared to club sailors (Figure 2B).
Central hemodynamic responses
Figure 3. displays changes in cardiac output, stroke volume, heart rate and systemic vascular conductance across the four hiking bouts between the two groups. During the four successive hiking bouts the increase from baseline in cardiac output (Figure 3A), and stroke volume (Figure 3B) were greater in highly-ranked sailors compared to club sailors (p<0.05). Whilst, systemic vascular conductance did not differ between groups across the three successive hiking bouts, at the limit of tolerance systemic vascular conductance was significantly greater in highly-ranked sailors (Figure 3D).
Figure 4. displays changes in systolic, diastolic and mean arterial blood pressure across the four hiking bouts between the two groups. The increase from baseline in systolic (Figure 4A), diastolic (Figure 4B) and mean arterial blood pressure (Figure 4C) across the four hiking bouts was greater in highly-ranked sailors compared to club sailors (p<0.05). 
Discussion
We investigated whether central hemodynamic responses and quadriceps local muscle oxygen availability during repetitive hiking bouts differ between sailors with different sailing ability according to the National Laser sailor�s ranking list. The novel findings of the present study are summarized as follows: 1) During three successive 2-min hiking bouts on a Laser-boat simulator, highly-ranked compared to less well-ranked sailors developed greater mean hiking moment. 2) In addition, highly-ranked sailors demonstrated greater stamina to endure a fourth hiking effort to the limit of tolerance compared to their less successful counterparts by exhibiting greater systemic vascular conductance and superior local muscle oxygen extraction capacity. 3) Such superior central and peripheral muscle capacities for oxygen transport and utilization may justify why highly-ranked sailors demonstrate advanced hiking ability. 
Compared to club sailors, highly-ranked sailors exhibited significantly greater muscle and total body mass (Table 1). This most likely justifies the finding that the latter category of sailors was able to develop greater maximal hiking moment as they were stronger and heavier than the former class of sailors. This observation is in line with earlier findings showing that the more successful sailors typically exhibit greater quadriceps maximal isometric force compared to club sailors (2,7).. In addition, highly-ranked sailors had more years of experience compared to club sailors (Table 1) which would be associated with more years of physical conditioning leading to superior development of their musculature and thus superior maximal quadriceps muscle force/moment. 
In the present study we used the same hiking apparatus and similar protocol that we employed in studies previously published by our group (7, 9, 11). This was deemed necessary in order to be able to reproduce and further expand our earlier findings on central hemodynamic and peripheral local muscle oxygen availability responses. Accordingly, the results of the present study confirm that hiking elicits significant elevation in both systolic and diastolic blood pressures (1,4,6,7,9) (Figure 4). In addition, the magnitude of change in cardiac output (Figure 3A) and quadriceps muscle deoxyhemoglobin concentration (Figure 2B) (often taken as an index of greater oxygen extraction) throughout the three successive hiking bouts in the present study is in line with that recently published by our group (10).
The novel feature of the present study is that highly-ranked compared to lower ranked sailors exhibited greater development in both systolic and diastolic blood pressures (Figure 4A and 4B) and superior capacity for peripheral local muscle oxygen extraction (Figure 2B). This is justified by the finding that the former group of sailors sustained the successive hiking manoeuvres at a greater absolute quadriceps muscle strength/moment (Figure 1B), most likely promoting the development of greater intramuscular pressures within the quadriceps muscles (8). Higher intramuscular quadriceps muscle pressures would impede blood flow within this muscle to a greater degree than in the muscle of the least successful sailors who most likely developed lower quadriceps muscle forces thereby, necessitating greater oxygen extraction compared to club sailors (23). 
The conception that top-class sailors possibly developed greater local intramuscular pressures during successive hiking bouts, is further supported by the local muscle oxygen availability measurements (Figure 2) displaying greater increase in quadriceps muscle deoxyhemoglobin concentration (Figure 2B) and greater reduction in quadriceps muscle oxygen saturation (Figure 2C) (23, 24). The latter situation would have occurred if blood flow through the quadriceps muscles was indeed more compromised in highly-ranked as compared top club sailors. In fact, this could have been the case as greater increase in deoxyhemoglobin concentration responses (Figure 2B) and greater reductions in both oxygenated hemoglobin concentration (Figure 2A) and quadriceps muscle oxygen saturation (Figure 2C) often occur during isometric contraction subsequently to greater restriction in local muscle blood flow (24). This suggestion is further supported by earlier findings from our laboratory (10) showing that in a mixed group of highly-ranked and club sailors, the progressive reduction in quadriceps muscle oxygen saturation during hiking was due to the progressive reduction in blood flow throughout these muscles. 
Despite the fact that top class sailors recruited their muscles to a similar fraction of their maximal force/moment, they sustained the isometric hiking efforts longer than their less well ranked-counterparts (298�10 versus 201�7 sec, respectively). This may probably explain what makes these sailors more successful in developing greater hiking moment. The prolonged endurance hiking time during the final all-out hiking effort confirms this suggestion.
The new feature of the present protocol compared to previously published studies from our group (9, 10) is that sailors were required to perform a final hiking effort to the limit of tolerance in order to identify potential differences in physiological variables at the point of hiking cessation. Accordingly, highly-ranked sailors were able to significantly prolong hiking endurance. At the end of the last hiking effort highly-ranked sailors demonstrated significantly greater systemic vascular conductance (Figure 3D) that it was associated with greater cardiac output (Figure 3A). Therefore, it is highly likely that elite sailors experience central cardiovascular and local muscle metabolic adaptations that facilitate oxygen transport and utilization during hiking, hence allowing them to sustain greater degrees of cardiovascular and, in particular, intramuscular strain. The latter local muscle adaptation suggests greater oxidative capacity (capillary network, mitochondria size, enzyme activity) on behalf of top-class sailors (25). 
A potential limitation of the present study is the lack of measurements of the fraction of quadriceps muscle integrated electromyography activity (% iEMG max) at which highly-ranked and club level sailors sustained the successive hiking efforts such recordings would have shade light to justify the greater cardiovascular and peripheral muscle oxygen extraction responses on behalf of the former group. However, based on findings of our previous studies employing the same Laser simulator and protocol utilized in this study, it is reasonable to assume that our assumptions have merit (6, 7). Furthermore, expressing the mean hiking moment as a fraction of the maximal moment developed on the toe-strap during a preliminary maximal effort could be used as a surrogate of the estimation of the fraction of MVC that the hiking bout was sustained. 
Conclusions
Highly-ranked sailors exhibit superior cardiovascular and quadriceps muscle oxygen utilization capacities. Such superior central and peripheral muscle capacities for oxygen transport and utilization may justify why highly-ranked sailors demonstrate advanced hiking ability.
