A rare ovarian pregnancy after intra uterine insemination procedure: A case report

Abstract
Background: Ovarian pregnancy is a very rare case. It is usually diagnosed during surgery. The incidence ranges from 1/2000 to 1/60.000 deliveries, approximately 3% of all ectopic pregnancies. Assisted Reproductive Technologies (ART) increases the risk of ectopic pregnancy compared with general population (0.8-8.6%) and ovarian pregnancy is 6% among all of the ectopic.
Case: This is a case report of 32-year-old nulliparous woman with ovarian pregnancy. She had primary infertility of 2 years due to Polycystic Ovarian Syndrome (PCOS). She underwent Controlled Ovarian Stimulation (COS) and Intra Uterine Insemination (IUI) procedure. 1 month after procedure, the urine pregnancy test was positive. However, she felt sudden sharp pain in the lower abdomen and suspected to have a ruptured ectopic pregnancy. Emergency exploratory laparotomy was performed. A ruptured left ovarian pregnancy was suspected during laparotomy, thus confirmed by histologic examination. Diagnosis of ovarian pregnancy is difficult without surgery due to its rarity and asymptomatic before rupture. The possibility of an ectopic pregnancy should be considered and informed to every couple who will undergo Artificial Reproductive Technologies, in this case an Intra Uterine Insemination procedure.
