Edge Trimming Phacoemulsification for Hard Nuclei

Abstract


Objectives: We describe a technique for phacoemulsification of hard (brunescent and white) cataract Methods: Interventional case series of 40 eyes of 37 patients with senile mature white or hard brunescent cataract. Phacoemulsification was done using edge trimming technique. The nucleus was sculpted in the center in a trial to divide it into 2 halves. When the nucleus was difficult(too hard, rubbery) to divide sculpting was completed in the form of wide deep saucer. After saucerization, the edge (equator) of the nucleus was lifted away from the capsular bag using Sinski hook. The edge of this equatorial zone was phacoemulsified while grasped between the phacotip &theSinski hook removing piece of the nucleus periphery, the nucleus then was rotated with Sinski hook to lift and emulsify the adjacent part of the edge of thenucleus and so on till the whole edge of the nucleus is removed leaving a central posterior plate that was elevated with the Sinski hook away from the capsular bag and emulsified. Foldable acrylic PCIOL was implanted. At the conclusion, Intracameral 0.1 ml moxifloxacin, 5 mg/ ml was injected. Results: In the 1st week 2 eyes developed transient corneal edema. Corneal phaco-burn developed in 1 eye that required single 10/0 nylon suture that was removed 1 month later. Visually insignificant iris phaco-burn occurred in 2 eyes.Posterior capsule rupture (PCR) without vitreous prolapse occurred in 1 eye. Endophthalmitis, IOL decentration or dropped lens fragments did not occur. Conclusion: Edge trimming phacoemulsification of hard cataract could be a safe alternative when the classic phaco-chop technique is difficult.
