The current evolution in Health Care Policy and emerging reimbursement models related to implementation of the Affordable Care Act has resulted in significant changes to care delivery practices. Patient-and family-centered care for the pediatric cancer patient in the home setting is expected to grow exponentially within the next decade. There is urgent need to develop models of care that insure optimal care delivery for this patient population with emphasis on care coordination, patient and family-centered care, and evidence-based practice in home-based care.
