Chronic diseases result in negative impacts on life quality, self-care ability and rise in morbidity and mortality risks which makes the importance of chronic disease management implementations, in the world and our country more significant. Management of chronic diseases includes not only treatment of patients, but also enabling patients to gain self-management ability, preparing and supporting them with various educational and training services to enable them to learn how to live together with their problems. Expectations of diseased individuals include, in addition to others, being continuously informed of their problems, ensuring continuity of their health care, being in continuous touch with, in particular, experts, re-arrangement/ acceleration of health-related procedures and being trained on how to cope with symptoms such as prostration, sleep disorders, depression, anger and stress. Attitude and knowledge of health care staff and use of appropriate techniques when dealing with patients have an important role in management of chronic diseases. Particular focus has been placed on the health care system in the studies in which factors complicating the management of chronic diseases were assessed. It has been reported that the most significant barrier against the disease management was caused by the health care system, a system that enables access to everybody and that comprehensive and coordination should be established. Considering the increase in frequency of chronic diseases in the near future, necessary measures and improvements against the mentioned barriers should be taken and made.
