Effect of cervical collar on oropharyngeal sealing pressure and fiberoptic view of the glottis with Ambu Aura Gain laryngeal airway: A randomized cross over trial

Abstract


Introduction: Laryngeal Mask Airways have been used for ventilation in patients with cervical collar in pre-hospital emergency settings and also in patients posted for various surgeries. Ambu Aura Gain is a newer LMA introduced recently Aim: To study the oropharyngeal sealing pressure and fiberoptic view of glottis with Ambu Aura GainTM laryngeal airway in patients with and without cervical collar Materials & Methods: Randomized cross over study. 35 healthy adults with normal airways were included in the study and Ambu Aura Gain LMA was inserted twice in same patient in a cross over fashion with collar and without collar. The oropharyngeal sealing pressure, fiberoptic view of glottis (Brimacombe score) along with insertion parameters were assessed. Analysis was done using paired t-test and Wilcoxen signed rank test. Results: The mean oropharyngeal sealing pressure in both the groups are similar no significant difference(29.60±3.77 without collar and 30.17±3.12 with collar(P-0.310)). The fiberoptic view of glottis was also similar in both groups(a median Brimacombe score of 4 in both the groups). The ease of insertion assessed by Likerts scale of 5 showed that with collar incidence of difficult insertion was more than without collar.(median score of 1 in non-collared patients and 2 in collared patients). Number of attempts were same in both groups(median of 1). Time taken to insert was significantly prolonged in with collar patients(without collar 21.34±9.67 seconds and with collar 26.17±11.71(P-0.025)) Conclusion: The Ambu Aura Gain LMA provides adequate oropharyngeal sealing pressure and ventilation in patients with cervical collar though the difficulty in insertion increases.

Suman Lata has completed the degrees of MBBS, DCH & MD (ANAESTHESIOLOGY) from Patna & Ranchi University RIMS School of Medicine, India. Presently she is working as Faculty in a premier organization JIPMER India. Dr. Suman has published more than 25 papers in reputed journals and also has been serving as a reviewer of reputed national journals. She has presented papers in national and international conferences. Also underwent short course in hospital administration from ministry of health and family welfare and training in transplant anaesthesia.
