The effectiveness of palliative care at home in terminal cancer patients

Abstract


Introduction: In the rural area, the medical source is lacked. The people may live alone or may be far away from hospital. The patients who are the terminal staged need more medical assistants. If some medical problems happened to them, such as the caring of the NG tube, foley care, the related education to the medicine, and pain control. It is difficult issue for them and need to be resolved. After our doctors` visiting, the patients did not need to admitted. By the suggestion of our doctor, we offer nursing homecare for them. Method: We went to the patients’ home, and try to know the kinds of disease that the patient were suffered. We survey that what kinds of the medical service is mandatory for the patients. The one of the goals is to decrease the cancer pain. Many patients live in the rural area, it is difficult for them to visit the hospital. We offer the medical information, helping to replace NG tube or Foley, Aromatherapy, Acupressure, caring the wound. We also teach the family membership how to care the patient. We help them to maintain the life ability and know medical knowledge. Result: Through 2016, we provided the related service to more than 200 oncology patients. We made the nervous patients calm down. The power of religion of Tzuchi was adopted, making they no more fear for the death. The psyco-social information and care were built. We had well preparation for the death. We also help the family to establish low emotion and condition followed up. If oncology patients were need to be admitted, the transfer assistants will be provided for them. Conclusion: The frequency of the home-care service is once a week . After receiving the home-care service, no matter in patients or their family, they all get highly satisfied . According to their contribution, through our service they can make themself relived, even in the alone period. The cost of home-care is not higher. The medical team we provide, including doctors, registered nurses, nutritionists, social workers. We had regulatory meeting about patient and family care. We design a plan to treat and care cancer patients. We sincerely hope that we can get more resources and invite specialist to join us. We can help more people in demand.

Yayu Ho, graduated from the Tzu chi university of Science and Technology in Taiwan, is a registered nurse and working at Family Medicine Department in Dalin Tzu Chi General Hospital in Chiayi. She is interested in homecare service among terminal cancer patients.
