Medical diagnosis (abbreviated Dx or DS) is that the process of determining which disease or condition explains an individual's symptoms and signs. It is most frequently mentioned as diagnosis with the medical context being implicit. The information required for diagnosis is usually collected from a history and physical examination of the person seeking medical aid . Often, one or more diagnostic procedures, like medical tests, also are done during the method.
