Efficacy of The Albumin-To-Fibrinogen Ratio as A Predictive Marker of Advanced-Stage Cervical Cancer

Abstract


Cervical cancer is the fourth most prevalent cause of mortality from cancer in women worldwide. Inflammation is a significant predictor of cancer. Inflammation is indicated by both a high fibrinogen and low albumin levels. Increased fibrinogen levels have been associated with the development and spread of cancer as well as pro-inflammatory diseases. The albumin/fibrinogen ratio (AFR), a combination of albumin and fibrinogen, has been proposed as a novel marker of inflammation. Therefore, this study aimed to evaluate the efficacy of the albumin-to-fibrinogen ratio as a predictive marker of advanced-stage cervical cancer. A prospective observational study was conducted in the Department of Gynecological Oncology at Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh, from July 2022 to June 2023. The study included 70 women with biopsy-confirmed cervical cancer. AFR values were compared between patients with early-stage and advanced-stage cervical cancer. The study findings reveal that AFR values were significantly lower (9.77±1.21) in patients with advanced-stage disease (FIGO stages III-IV) compared to early-stage (FIGO stages I-II) cases (10.74±1.54) (p=0.005). Considering 10.7 as the AFR cutoff, the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) for predicting advanced-stage cervical cancer were 85.71%, 62.86%, 69.77%, and 81.48%, respectively. In conclusion, AFR is a potential noninvasive marker that can distinguish between the early and advanced stages of cervical cancer. Its application in clinical practice may enhance decision-making and patient management.

She completed her MS in Gynaecological Oncology from Bangabandhu Sheikh Mujib Medical University, Bangladesh. She has over 15 publications that have been published in recognized indexed and PubMed-indexed journals. She worked as a medical officer at the Gynaecology & Obstetrics department of Marks Medical College & Hospital for a year. Then, she also worked as a resident at Bangabandhu Sheikh Mujib Medical University, Bangladesh for 5 years. She is now working as a consultant at New Al-Razi Hospital, Mawna, Gazipur, Bangladesh.
