Tubeless operation for upper airway surgery using STRIVE Hi in pediatrics

Abstract
Pediatric airway management remains the most daunting task before the anesthesiologist. SponTaneous Respiration using Intra VEnous anesthesia and Highflow nasal oxygen (STRIVE Hi) is a recently developed, spontaneous ventilation open airway technique, which facilitates microlaryngoscopy and endoscopic airway surgery in pediatric age group. The STRIVE Hi in pediatrics utilizes an upwardly titrated infusion of Propofol, with the addition of Dexmedetomidine infusion, in combination with high-flow nasal Cannula (HFNC) delivery at one l/Kg /min. The presented videos and presentation demonstrate good operating conditions and absent airway reactivity to surgical manipulation. In most cases, the vocal cords were abducted and immobile or associated with minimal movement, which did not interfere with the surgical procedure. Case 1: shows surgical removal of recurrent vocal cord polyposis using surgical debridement of papilloma done using micro shaver 2.9 blade and vocal cords was abducted and minimally moving. Case 2: shows surgical removal of vocal cord web release the web with cold dissection and suturing the flap in the left vocal cord with 2 sutures, Case 3: shows surgical repair of laryngeal cleft in coffin-sirissyndrome; Laser was used to resurface the laryngeal cleft and suturing of raw area and the vocal cord was minimally moving.
Biography
Nabil Abdelhamid Shallik was trained in Faculty of Medicine, Tanta University, Egypt and worked as anaesthesiologist at Tanta University Hospitals, in 1993. After completion of his Medical Doctorate Degree (MD) thesis in coagulation in paediatric open heart surgery, he employed as Lecturer in anaesthesia department, Tanta University Hospitals in 2002. He was promoted as Assistant Professor in same department in 2007. He became also an assistant professor in Weill Cornell Medical College in Qatar, from June, 2009 up till now. He is in several subcommittees of the European Airway Management Society (EAMS), international teacher in Airway Management Academy (AMA), Publications Committee board member of Pan Arab Federation of Societies of Anesthesia, Intensive Care and Pain Management (PAFSA), Society of Airway Management (SAM) and Difficult Airway Society (DAS), The European Society of Regional Anesthesia and Pain Therapy (ESRA) and The Egyptian Society of Anesthesia and Intensive care, plus he is the director of airway courses at Hamad Medical Education
