Tuberculoma in a neonate with associated umbilical vein aneurysm-a case report

Abstract


Intracranial tuberculoma in new born is a rare occurrence. It is also a diagnostic difficulty due to its rarity and atypical clinical manifestations. We report a one week 2,500 grams neonate born from a para-ii mother at a gestational age of 35 weeks in Felege Hiwot Hospital. The mother had three antenatal follow up during which she was diagnosed as new TB case and started treatment three months prior to her delivery. Antenatal Obstetric Ultrasound showed normal pregnancy. A week prior to the delivery, she started to have intermittent vaginal bleeding. Repeated obstetric ultrasound showed placenta posterior and fundal and no sign of abruption and multiple well defined masses on the umbilical cord. Meanwhile the baby’s condition gets deteriorated, emergency cesarean section was done to deliver a 2,500 grams male with APGAR of 5 and 6 in the 1st and 5th minutes, with widened anterior fontanel, increased tones of lower extremities and progressively fails to suck. A serious investigations post-partum concluded that intracranial tuberculoma, with associated umbilical vein aneurysms. The diagnosis, treatment outcome and follow up of the case will be discussed in the presentation.
