Case of Opsoclonus Myoclonus Ataxia Syndrome with Benign Ovarian Teratoma

Aim: To study a case of opsoclonus myoclonus ataxia syndrome in respect to aetiology, treatment and outcome in young female.

Results: 26 years female presented with imbalance during walking since last 1 week with episodic jerky movements all over body with oscillopsia. On examination opsoclonus, cortical myoclonus and appendicular and truncal ataxia.

Her MRI brain, CSF routine, CSF NMDA were negative. She was started on IVIG. Her whole-body PET scan was suggestive of right ovarian teratoma so urgently she was taken for laparoscopic excision of teratoma is done. Histopathology was suggestive of benign teratoma.

She was kept on oral prednisolone for three months. After tapering she developed again ataxia and opsoclonus. Evaluation was negative for residual teratoma. Her serum NMDA antibodies were positive. She is started on rituximab she has shown significant improvement.

Conclusions: Opsoclonus myoclonus ataxia syndrome due to ovarian teratoma is rare, treatment is surgical excision and immunosuppressive therapy. All young females with opsoclonus myoclonus ataxia syndrome, should thoroughly evaluated for underlying ovarian teratoma.
