We present a case of spinal epidural hematoma after Lumbar catheter insertion in a case that passed TEVAR for an aneurysm of the descending aorta. Spinal epidural hematoma (SEH) is a fairly rare reality and has been reported to do in cases who admiralty-coagulant remedies, have bleeding diseases, or after traumatic needle insertion. In our case, the epidural hematoma passed 4 hours after catheter insertion, in a case that entered anticoagulant and antiplatelet drug.

In these cases, early discovery of symptoms and rapid-fire opinion are an imperative. Our case’s opinion was done 5 hours after symptom development and it comported of thoracolumbar MRI that showed a spinal hyperactive-acute epidural hematoma extending from T9-T10 to L4-L5 and the affected parts of the spinal cord and conus medullar showed edema, utmost significantly at L1-L2levels.A neurosurgical consult was done incontinently after carrying the results and within six hours after neurosurgical symptoms passed, the case passed spinal relaxation, the hematoma was vacated and the case recaptured motor function in her legs. During her stay at the cardiac ICU, the croakers conducted to our case antiplatelet drug alongside low-molecular- weight heparin (LMWH) – inj. Cleaned 40I.U., against our recommendations. Due to this, our patient advancer-bleeding and we had to perform a alternate surgery, to do a more expansive spinal relaxation and to void the hematoma. Neurologic recuperation after the alternate surgery was delicate, but with expansive physical remedy, we've managed to achieve some enhancement, with motor response 2 5 on the left leg and 3 5 on the right leg. Although anticoagulants and or antiplatelet drug are a must-have in endovascular procedures and are considered safe to use during epidural analgesia, special attention and care to epidural hematoma should be given, especially in cases when an epidural catheter placement is demanded.

Spinal Epidural Hematoma (SEH) is a fairly rare reality first described by Jackson. Spinal hematoma is the accumulation of blood in the implicit space between the bones. It can be a complication of neuroaxial an aesthesia ways, especially in those cases with a crazed coagulation profile due to systemic conditions (e.g. hepatic conditions, renal failure) or anticoagulant remedy. It's more common in the cases treated with anticoagulants, thrombocytopenia, or in cases with alcoholic liverdisease.There have been numerous studies that have described the circumstance of spinal epidural hematomas associated with the placement and junking of Lumbar catheters. Due to the association between the duration of hematoma- convinced spinal cord contraction and the degree of neurological symptoms, prompt opinion and surgical treatment are critically important determinants of neurological recovery. As the progression of severe symptoms generally occurs after an epidural catheter has been placed for further than 6 hours.

Upon admission at the cardiac surgery sect, she was fixed and passed surgery to perform a subclavian to carotid transposition (SCT) as an adjunct for TEVAR. The surgical procedure passed without any complications and she was admitted at the University Clinic of Cardiology for farther treatment. Pre intervention bold work-up showed a normal CBC, biochemical profile showed normal liver function tests, urea and cretonne were within normal values, glucose situations were9.51 mole/ L, cardiac enzymes were within normal ranges and contagion labels were negative. Cardiac ultrasonography revealed normal confines of the left ventricle with normal global systolic function, moderate hypertrophy of the interventricular septum, slightly enlarged left patio, right patio and ventricle within normal confines, valvulary outfit of the heart normal according to age, no pericardial effusion and dilated thrusting aorta up to 40 mm. ECG-Gated Cardiac CT was performed that showed a fusiform aneurysm of the descending aorta with minimal periphery lower than 6 cm. Then we're presenting a case of spinal epidural hematoma after Lumbar catheter insertion in a case who passed TEVAR for an aneurysm of the descending aorta. After all the necessarypre-interventional analysis the case passed TEVAR procedure. Before the procedure an Lumbar drain was placed to drain at 10 mm Hg (15 cm H2O) for 24 hours. The case during the intervention was in supine position, epidural anesthesia was administered and access was gained through the femoral tone and a Valiant ™ thoracic stent graft with the Captives ™ delivery system was fitted and the aneurysm was repaired. During the procedure, an occlusion of the left common carotid roadway passed and a Dynamic8.0 x56 mm stent was placed. The procedure when without any farther complications and the case was transferred at the cardiac ICU for farther observation and treatment. After the procedure, the case entered 600 mg of Clopidogrel and 100 mg of Aspirin. Six hours after the intervention, the case complained about pain in her lower extremities with dropped capability to move. There was blood in her Lumbar drainage catheter. The original check up from the anesthesiologist revealed a dropped motor function on the lower extremities, and MRI of the lumbar chine was ordered and corticosteroid remedy was initiated and the Lumbar drainage catheter was removed.

After carrying the MRI results, the cardiologist asked for a neurosurgical consult. Neurological status revealed motor response 1 5 on the left leg indicating plague of the extremity and 2 5 on the right leg with minimum movements – severe paresis, dropped sensibility to touch, more pronounced on the left leg. An imperative suggestion for surgery was put forth and the case was transferred at the PHO University Clinic of Neurosurgery Mother Teresa, Skopje. Afterpiece-op medication, the case was brought to the operation room, intubated and deposited in a modified genu-pectoral position. After careful medication and draping of the surgical field, we performed a midline gash of the skin on the lumbar region of the chine, progressed with careful analysis of the soft napkins, at L3 position there were towel blights harmonious with multiple perforation in order to place the Lumbar drainage. A double laminectomy L2 and L3 situations was performed, the epidural hematoma was successfully vacated and original hemostasis was achieved, a drain was placed and the soft napkins were stitched consequently.
